Pre-Registration and Immunization Clinic
WOISD invites you to come to a districtwide pre-registration and an immunization clinic sponsored by the Texas Department of Health Services.
 
Date Wednesday, August 4
Time: 8:30 a.m. - 11:30 a.m.
1:00 p.m. -    3:30 p.m.
Location: West Oso Elementary School
1526 Cliff Maus Drive
Corpus Christi, Texas 78416

 
Please bring the following documentation:
  • End-of-Year Report Card
  • Birth Certificate
  • Social Security Card
  • Proof of Residency (utility bill or copy of lease or home-buying contract)
  • Valid Identification of parent or guardian
  • Power of Attorney or court order if someone other than parent is enrolling student

These documents are required for registration. If your child is enrolled in the CHIPS program or receives Medicaid, please bring card.  There will be a service fee of $ 5.00 per child or $ 10.00 per family. No child will be refused services.
