Pre-Registration and Immunization Clinic
WOISD invites you to come to a districtwide pre-registration and an immunization clinic sponsored by the Texas Department of Health Services.
Immunization clinic:
Date Wednesday, August 4
Time: 8:30 a.m. -   3:30 p.m.
Location: West Oso Elementary School
1526 Cliff Maus Drive
Corpus Christi, Texas 78416

 
Preregistration for newcomers will be held at each of the corresponding campuses starting  August 4, 2005.

Please bring the following documentation:

  • End-of-Year Report Card
  • Birth Certificate
  • Social Security Card
  • Proof of Residency (utility bill or copy of lease or home-buying contract)
  • Valid Identification of parent or guardian
  • Power of Attorney or court order if someone other than parent is enrolling student
  • Immunization Record
  • Record and/or transcript from incoming district
  • Valid identification of parent or guardian
  • Power of attorney document or court order  (if other than parent )
  • Proof of income

Grades 9 - 12

  • August 3,  from 8 am - 11:30 am - students new to the district
  • August 10, form 8 a.m. - 11:30 pm.. - Juniors, registration/schedule distribution.
  • August 11, form 8 a.m. - 11:30 p.m. - Sophomores, registration/schedule distribution.
  • August 11, form 1 p.m. - 3:30 p.m. - Juniors, registration/schedule distribution.
