West Oso Independent School District invites you to come to a district-wide Registration and an Immunization Clinic sponsored by the Corpus Christi Health Department.

Date: Thursday, August 2, 2007
Time: 8:30 - 11:30 am and 1:00 - 3:30 pm
Location: West Oso Elementary School. 1526 Cliff Maus Dr. Corpus Christi, TX 78416
 

Please bring the following documentation:

• End of year Report Card
• Birth Certificate
• Social Security Card
• Immunization Record
• Proof of Residency (utility bill or copy of lease or home buying contract)
• Valid identification of parent or guardian
• Power of Attorney or court order if someone their than parent is enrolling student.

These documents are required for registration. If you child is enrolled in the Chips program receives Medicaid, please bring current card.  There will be a service fee of $5.00 per child or $10.00 per family.  No child will be refused services.
