West Oso Independent School District invites you to come to pre-registration and an immunization clinic sponsored by the Corpus Christi Nueces County Health Department.

Important:
Your child may not be able to attend school if they have not been fully immunized before school starts on Monday, August 14.

For registration, please bring the following documentation:

• End of year report card

• Original birth certificate

• Social Security card of child being enrolled

• Current immunization record

• Proof of Residency (ex. Current utility bill, light, water, gas) or a copy of lease or home buying contract with the name of the person enrolling student.

• Valid identification of parent or guardian (ex. government-issued driver's license, identification card with picture)

• Power of attorney or court order if someone other than parent is enrolling student.

• Proof of income (ex. Current check stub, government check, or letter from DHS for assistance offered).

These documents are required for registration.  If you child is enrolled in the CHIPS program or receives Medicaid, please bring card.
There will be a service fee of $ 5 per child or $ 10 per family.  No child will be refused services.
